Patient Resources //

In order to book a new patient appointment with our office you will need to have your primary care doctor or another specialist refer you to us. We ask that you have them send us a referral with your current medical records and bloodwork via Epic or Fax to 860-430-9770. Once received by our office we will have the physicians review your documents and we will call you to schedule. Please be sure that if your insurance requires a referral to see a specialist, your primary care doctor has one in place for all your visits to our office.

We are currently accepting patients with and are credentialed with all insurance companies EXCEPT the following:

-Anthem High Performance Network

-Medicaid/Husky

-Aetna Trinity Health Plan (yellow card)

-Cigna Medicare Advantage as of 11/1/2024
